Quiste Hepático Gigante Tratado mediante Cistectomía Laparoscópica con Hallazgo Inesperado de Macroadenoma Hipofisiario Hemorrágico Manifestado como Vértigo Postoperatorio: Reporte de Caso

Las lesiones quísticas hepáticas son entidades frecuentes en la población adulta mayor, generalmente asintomáticas, pero potencialmente sintomáticas cuando alcanzan gran tamaño, requiriendo manejo quirúrgico. En el periodo postoperatorio, la aparición de vértigo suele atribuirse a causas benignas; sin embargo, en determinados contextos puede constituir la manifestación inicial de patologías neurológicas centrales subyacentes, incluyendo lesiones hipofisiarias complicadas. Se reporta el caso de una paciente femenina de 84 años con antecedentes de hipertensión arterial e hipotiroidismo, quien ingresó por masa hepática sintomática. Fue sometida a cistectomía hepática laparoscópica, evidenciándose intraoperatoriamente quistes en lóbulo hepático izquierdo de hasta 17 × 11 cm, realizándose drenaje y resección capsular sin complicaciones. En el postoperatorio, la paciente presentó episodios de vértigo, ortostatismo y diaforesis, asociados a epigastralgia y leve déficit motor en hemicuerpo derecho. El electrocardiograma evidenció intervalo QTc prolongado y alteraciones inespecíficas de la repolarización. Ante la persistencia de síntomas neurológicos, se realizó resonancia magnética cerebral, que reveló un macro adenoma hipofisiario con contenido sugestivo de hemorragia intralesional, además de cambios crónicos compatibles con microangiopatía cerebral. La paciente fue manejada de forma conservadora con tratamiento sintomático y seguimiento multidisciplinario, presentando evolución clínica favorable con resolución progresiva del vértigo. El vértigo en el postoperatorio de cirugía abdominal en pacientes geriátricos no debe considerarse exclusivamente de origen benigno. Este caso resalta la importancia de un enfoque diagnóstico integral, ya que puede representar la manifestación de patologías neurológicas centrales potencialmente graves, como la apoplejía hipofisiaria. La identificación oportuna y el manejo multidisciplinario son fundamentales para optimizar los desenlaces clínicos.
